Measles in the US: 5,182 Cases, and Elimination Status on the Line Next Week

The United States has recorded 5,182 confirmed measles cases in 2026, the Centers for Disease Control and Prevention reported on Friday — the highest annual total in 35 years, and more than double the 2,289 cases recorded in all of 2025. The count, current through November 5, rose by 330 cases in a single week, the 13th consecutive week of triple-digit growth.

Ninety-six percent of this year's cases were in people who were unvaccinated or whose vaccination status was unknown. The disease has now reached 45 states and Washington, D.C.; only Arkansas, Hawaii, Mississippi, Nevada and New Hampshire have reported none. Pennsylvania is the hardest-hit state, followed by South Carolina with 670 cases and Utah with 536. Wisconsin's state count has reached 477, its highest in 35 years.

The human cost is no longer abstract. The CDC counts four measles-associated deaths in 2026; state health departments have reported at least eight, including three in a single week: an infant in Wisconsin, a resident of Pennsylvania, and a child in Lewis County, New York. Wisconsin's was the state's first measles death in more than 35 years; New York's its first in 30. Another 562 people — 11 percent of cases — have been hospitalized. The death-count gap itself has become part of the story: under Health Secretary Robert F. Kennedy Jr., the CDC switched from state health departments' figures to mortality data from the National Center for Health Statistics, and says it is still developing a standardized case definition.

Measles was declared eliminated in the United States in 2000, meaning the virus no longer spread continuously inside the country. That status now faces its formal test. A commission of the Pan American Health Organization meets November 10–13 to review whether the United States still qualifies. The test is genetic, not numerical: PAHO defines re-established endemic transmission as the uninterrupted circulation of the same virus genotype and lineage for 12 months or more, and it set January 20, 2025 — the start of the West Texas outbreak — as the beginning of the American analysis period. Canada lost its own elimination status under the same standard in November 2025, which also cost the Americas its regional measles-free designation.

The review was originally scheduled for April and was pushed to November while American authorities completed analysis, including full viral genome sequencing. The commission's meeting is closed, and it does not announce a ruling itself; it sends recommendations to PAHO's director, who formally communicates the classification to national authorities, so a public verdict may lag the meeting by days or weeks.

What is at stake is not a label. Elimination status is the public-health world's way of saying a country has broken the chain — that every case can be traced to an import, and that the virus cannot sustain itself. Losing it would be the official acknowledgement that measles, a disease the United States defeated a generation ago and could have kept defeated with a two-dose vaccine that costs a few dollars, has learned to live here again. The vaccine has not changed. The virus has not changed. What changed is how many people are willing to take it.

What to watch now is the closed-door meeting in Washington next week, and whether the genomic evidence shows one unbroken chain of transmission stretching back to January 2025. If it does, the United States will join Canada in losing a status it held for 26 years — and the next question will be how long it takes to win it back.

5,182 cases, and the review that decides elimination status arrives next week — the timing is almost theatrical. Elimination status was never a trophy; it is a promise that any outbreak anywhere gets stamped out before it spreads everywhere. Whether the review keeps the label or takes it away, the case count itself is the real verdict.
